CONSULTATION FORM

I’m excited to work with you! Please fill out the form to the best of your ability. There is no upload form for photos, so please hold on to your references. Once I begin corresponding with you, I will ask for those references and you can send them over.

Please note that if you are interested in a flash piece that I designed, a contact form is not necessary. Just email me directly or call the shop (ask for Mel) so that I can find you a date and time.

Please describe in a few short sentences the design you are looking to get done. Once you have submitted, Please allow 2-7 days for a response (in case I’m having a busy week) I typically answer emails once a day, work Thurs-Sun 12-6pm, and I only book 1-2 months in advance. Deposit of $100 is required to hold your date, which is only done after we have decided on a date, time, and price range. If we are discussing a potential date, please respond promptly(3 days) or risk losing your place if a deposit is not made. Transparency is appreciated if you need more time to make the appointment/deposit at any time during our consultation. More will be explained during the email/consultation process.

A design is not shown until the day of the appointment, but I encourage in-person consultations so that we can sketch out your design. I understand some people aren’t comfortable with that, you can always reach out to me so that we can seek an alternative to our project together.
